Please include your name, recipient's name, dollar amount, and date you would like to pick up your request. If you would like it mailed directly to the recipient, please include that mailing address in your request. Also include a contact number so we may arrange a time to process your transaction.

Cancellation Policy
To all new and existing patients:
Please take note that due to the time constraints of our practitioners' and therapists' schedules, we ask that if you need to either cancel or re-schedule your appointment, it must be done at least 24 HOURS BEFORE YOUR SCHEDULED APPOINTMENT. Please understand the enormity of this policy.
If the appointment is adjusted or canceled within that 24 hour time frame (unless it is in case of emergency or unique circumstances) - you will be charged for 50% of the treatment cost on your next visit.
If you book an appointment within 24 hours of your treatment - it is considered confirmed at the time of the booking.

Privacy Policy
All information in the patient's file will be kept confidential, although the file will be shared among the treating therapists in this facility. The sharing of the file will improve the flow of information among the professionals to ensure the utmost quality of care.
The patient understands that written authorization will be obtained prior to any release of information, except when required by a court of law.
